
Thank you for taking the next step towards scheduling an evaluation at Pawlished Dog Training. Please read the following, in its entirety, before proceeding. We have been making program changes that you should be made aware of before completing this form.
Evaluation Policy Update:
We require all new clients to complete an evaluation before beginning any training program. The evaluation allows us to meet you and your dog, assess behavior, discuss goals, and recommend a training plan tailored to your needs. Previously, this has always been a free service. Any evaluations booked after September 1 2026 will have a booking fee required.
Booking Fee Details:
- A non-refundable $75 booking fee is required at the time of scheduling to secure your appointment.
- This fee will be applied toward the cost of any training program booked after your evaluation.
- If you cancel within 48-hours or miss your appointment, this fee will not be refunded or credited to a future date.
Rescheduling & Cancellations:
- You may reschedule your evaluation once with at least 48 hours’ notice.
- Reschedules with less than 48 hours’ notice or multiple changes may result in forfeiture of your booking fee.
- No-shows are considered cancellations and will not be eligible for a refund or transfer of the booking fee.
Dog Eligibility:
- All dogs must be up to date on age-appropriate vaccinations and in good general health for their appointment.
- If your dog is showing signs of illness (coughing, diarrhea, vomiting, etc.), please contact us to reschedule.
- Dogs with a bite history or significant behavioral concerns must be disclosed on the request form prior to scheduling.
- Pawlished does not accept human aggressive dogs. This includes dogs who have bitten humans inside or outside of the household and dogs who have been quarantined for a bite to a human.
Arrival & Safety:
- Please arrive on time and keep your dog on a secure collar/harness and leash.
- If your dog is reactive or nervous, we recommend waiting in your car and texting us upon arrival.
- For safety, we do not allow dog-to-dog greetings on-site unless directed by a trainer.
What to Expect:
- Evaluations typically last 45–60 minutes.
- We’ll discuss your goals, assess your dog’s behavior, and explain which of our programs are a good fit.
- You are under no obligation to sign up for training at the time of your evaluation.
FAQs:
- The Norwood location is permanently closed. All evaluations and initial training will take place in Mentor, Kentucky.
- Pawlished Dog Training requires dogs to complete their Private Lesson or Residency Programs before joining us for graduate events like group classes, pack walks, and workshops.
The following form covers the basics of your dog's life, so that we can dig right into behavioral questions during your evaluation. Pawlished is relying on this information to train you and your dog. Please be honest and give detailed answers. Skipping questions, giving short, incomplete answers, and rushing through this form is not beneficial to either of us. It is long and detailed, so that Pawlished can get an idea of what your life looks like with your dog, what behavioral concerns you have, and what goals you’d like to work towards.
Please complete individual forms for each dog in your household that you wish to have evaluated.
Once you have completed this form, you will receive an email from Tori (pawlisheddogtraining@gmail.com) with available appointment times based on your answers within 72 hours.
